Case study procedures and guidelines

 PROCEDURE AND GUIDELINES FOR SUBMITTING CONNEXIONS CASE STUDIES


1. Case studies must be submitted to the Connexions Performance Improvement Manager by the end of the month assigned to your organisation in the schedule (Annex 1) using the standard proforma (Annex 2).

2. Case studies must be accompanied by the original consent agreement, signed by both the young person and their parent or carer (Annex 3).

3. All case studies must be signed and dated by the original author or their line manager and should identify which organisation is represented (Annex 2).

4. The originating author, or their line manager, should identify a theme for the study (see Annex 4) and include on the standard reporting form.

5. Because of the consent requirement the case study must be sent in both electronic and hard copy formats.

6. The originating partner must keep a copy of the agreement. This should be attached to the young person's case notes/ records.

7. Case studies should be no longer than 500 words long. (Annex 2b provides an example of a case study)

8. All case studies should be anonymised, preferably using a single character, e.g. J

9. Connexions Hertfordshire will submit one case study every 6 weeks to CSNU case.studies@dfes.gsi.gov.uk

10. Connexions Hertfordshire will ensure that a fair cross section of case studies will be submitted and notify the originating partner organisation.

11. Case studies will be held centrally for 12 months and then destroyed.

12. The procedure will be reviewed annually.
